Band Fallopian Tubes Ke Baad IVF + FET Se Pregnancy – Case Study

The blockage of the fallopian tubes is one of the biggest obstacles to natural conception because the egg and sperm are unable to come together. This case study is an account of how one 30-year-old female patient in our Prime IVF center has experienced successful IVF 2 IVF + FET treatment cycles in only 2-3 months. Our fertility specialists, embryologists, and gynecologists at Prime IVF centre, recognized as the Best IVF Centre in Gurgaon, carefully handled complicated infertility cases.

Patient’s medical history and fertility challenges

As explained beforehand, our patient has been married for 10 years and has undergone multiple unsuccessful attempts to conceive in the last 8 years. Our patient complained of irregular menstrual cycles and that she had already undergone fertility treatments, resulting in failed results. Her medical history showed that she had bilateral tubal blockage, which impaired natural conception to a considerable extent. Her AMH level of 1.02 ng/mL was relatively low in relation to her age, and thus, timely and well-outlined intervention was necessary to increase her likelihood of pregnancy.

We critically examined how the medical history of this patient gradually became an issue of severe fertility problems. Her irregular menstrual cycles that had long existed were not merely a symptom and a prediction of potential ovulatory imbalance. Abnormal ovulation has directly impacted the maturation and release of eggs and decreased the possibility of natural conception each month. She has a low AMH level, and her fallopian tubes were blocked.

Also, the abnormal parameters of semen were a contributing factor to the problem. The couple was not experiencing just one problem, but was not seeing promising results due to several overlapping causes. A combination of these factors turned her medical history into a multifactorial and complex infertility condition with high-level and timely reproductive needs.

Diagnostic Findings and Key Test Results

We held a rigorous diagnostic analysis to clearly comprehend the disorder behind infertility and direct the most suitable treatment course. Particular emphasis was placed on utilizing ultrasound imaging and hysterosalpingography (HSG) to evaluate ovarian function and the condition of the fallopian tubes. We had done a transvaginal ultrasound to evaluate ovarian reserve and follicular activity and uterine health. The scan revealed follicular growth in line with her AMH level of 1.02 ng/mL, which is relatively low and consistent with diminished ovarian reserve. The uterine cavity was structurally normal, and there was sufficient endometrial thickness, indicating an effective potential for embryo implantation. No significant fibroids, polyps, or structural abnormalities that might interfere with embryo transfer were found.

Nevertheless, the Hysterosalpingography (HSG) test gave a great discovery: a band within the fallopian tube. This organizational problem, over time, also made her infertility process a long one. The AMH level of 1.02 ng/mL was also a sign of low ovarian reserve when compared to the normal range at her age. Following this, we decided that a delayed intervention might further decrease the number of eggs that could be retrieved, limiting the choices of treatment.

IVF + FET Plan of Treatment

Following a comprehensive analysis of the diagnostic data—which revealed bilateral tubal occlusion, ovulatory dysfunction, diminished ovarian reserve ($AMH = 1.02$ ng/mL), and male factor infertility – Prime IVF implemented a structured protocol for IVF and Frozen Embryo Transfer (FET). This was a plan that would circumvent the blocked tubes, correct sperm abnormalities, and provide optimal conditions of implantation. This strategy was designed to facilitate a successful pregnancy within an expedited timeframe of approximately two to three months.

Ovarian Stimulation and Egg Retrieval

We were treated with controlled ovarian stimulation. We used the individualized hormonal injections that triggered more than one follicle per cycle, and this was well monitored using serial ultrasounds and hormonal testing. Her ovarian reserve was moderately low and therefore, we adjusted medication dosages to the maximum to ensure optimal response without affecting egg quality.

We followed up closely on her follicular growth, and when the follicle was at a good size, a trigger injection was given to stimulate final egg development. We extracted mature eggs with the help of transvaginal ultrasound and mild sedation.

We were able to isolate 5-6 mature oocytes. This was satisfactory based on her AMH level. Each egg that was retrieved was examined at once by our team of embryologists to determine if it was mature enough and capable of fertilization.

Fertilization and Development of the Embryo

Since there were abnormal semen parameters, we optimised advanced fertilisation techniques in order to enhance the success rate. Our embryology team selected the healthiest sperm to be fertilized to get the best embryo.

The patient’s fertilization was successful, and the embryos were cultured in our highly controlled laboratory conditions. The constant check-up enabled our embryologists to determine cell division, symmetry, and general progression of development. Positively, embryos of an A-grade were obtained, which signifies a high potential for development and good morphology.

As our patient’s uterine environment is effectively optimized in a later cycle, a Frozen Embryo Transfer (FET) was performed. This gave our patient the required time to return to its pre-stimulation state, and the endometrium was in the perfect conditions for implantation due to the hormonal injections we used.

Embryo Transfer (FET)

To monitor the endometrium, we used ultrasound in the prepared FET cycle and ensured that it was thick enough and receptive. Hormonal support was given to align the embryo’s growth with the uterine lining.

The embryo transfer was carried out on the 19 th and 20 th day of the cycle. We transferred three good embryos with utmost precision to the uterus. Our process was painless and did not necessitate anesthesia, and it was over a couple of hours.

We monitored carefully injected progesterone and observed after transfer. Our patient was recommended to take a break, have a healthy lifestyle, and follow up therapy on the scheduled visits.

Pregnancy Test Result and Confirmation

About 12-14 days after embryo transfer, we conducted a Beta HCG blood test to verify pregnancy. The outcome of our patient was successful which is a huge step in the long fertility process of the couple.

Our ultrasound examination later on revealed an intrauterine pregnancy, which proved successful implantation and early fetal development. This was particularly significant considering the compound fertility issues, i.e., tubal blockage, ovarian reserve issues, irregular cycles, and male factor infertility.

Even though the case is complicated, our well-designed IVF + FET protocol, tailored monitoring, and embryology assistance served as the keys to changing a challenging clinical scenario into a happy and satisfying conclusion.
